So one could say β€οΈπŸ’­πŸ•ŠοΈβž‘οΈπŸ₯°πŸ›ΆπŸŒŠ and it could mean "The heart's dreams fly to its beloved one as they sail upon the sea," or "Love's thoughts soar to one who sails among the waves," or something along those lines, depending on how it is used? Is there a way to differentiate, or is the interpretation up to the reader?

It can both matter and not matter, depending on your position and how one theoretically desires to both express themself and be understood.

Poetry can be viewed from many different angles, and it's the process of trying to see it through the eyes of the artist or the reader that creates the varying emotional reactions. If I view a poem through my own eyes in my natural state, I will see it differently than if I try and put myself in the shoes of someone who is in a situation that is different than my own. That is also why a person's reaction to a poem or any sort of art will vary widely, hence rendering right and wrong meaningless.
